Manage Patient Cohorts

Manage Patient Cohorts is the area of Accuro where a user can set up named cohorts and select who is to be included in the group.

Access Manage Patient Cohorts

1. Click the Patients button on the Accuro Navigation bar.
2. Click the Demographics tab.
3. Click the Manage Cohorts button located at the bottom of the window. The Manage Patient Cohorts window is displayed.

Add a Cohort

1. Click the Patients button on the Accuro Navigation bar.
2. Click the Demographics tab.
3. Click the Manage Cohorts button located at the bottom of the window. The Manage Patient Cohorts window is displayed.
   
   
4. Select the appropriate Provider.
5. Click the Add button in the Cohort section. The Add Patient Cohort window is displayed:
   
   
6. Type in the cohort name and click OK to save.

Edit a Cohort

1. Click the Patients button on the Accuro Navigation bar.
2. Click the Demographics tab.
3. Click the Manage Cohorts button located at the bottom of the window. The Manage Patient Cohorts window is displayed.
   
   
4. Select the appropriate Provider.
5. Click the Edit button in the Cohort section. The Edit Patient Cohort window is displayed:
   
   
6. Type in the new cohort name and click OK to save.

Remove a Cohort

1. Click the Patients button on the Accuro Navigation bar.
2. Click the Demographics tab.
3. Click the Manage Cohorts button located at the bottom of the window. The Manage Patient Cohorts window is displayed.
   
   
4. Select the appropriate Provider.
5. Click to select the cohort being removed.
6. Click the Remove button in the Cohort section. A confirmation message is displayed:
   
   
7. Click Yes to delete the cohort.

Add Patients to a Cohort

1. Click the Patients button on the Accuro Navigation bar.
2. Click the Demographics tab.
3. Click the Manage Cohorts button located at the bottom of the window. The Manage Patient Cohorts window is displayed.
   
   
4. Select the appropriate Provider.
5. Click to select the cohort in the top section.
6. Click the Add button in the Patients of Cohort section. The Patient Search window is displayed:
   
   
7. Search for the patient being added to the cohort and click OK to save.
   
   Note: To add an patient that is new to the clinic, click the New Patient button.

Remove Patients from a Cohort

1. Click the Patients button on the Accuro Navigation bar.
2. Click the Demographics tab.
3. Click the Manage Cohorts button located at the bottom of the window. The Manage Patient Cohorts window is displayed.
   
   
4. Select the appropriate Provider.
5. Click to select the cohort in the top section.
6. Click to select the patient being removed from the cohort in the bottom section.
7. Click the Remove button in the Patients of Cohort section. The patient is removed from the cohort
   
   Note: There is no warning message before the patient is removed from the cohort.
