Chronic Conditions

Chronic Conditions are conditions that a patient will have for a long time (often the rest of his or her life). Some examples include diabetes, coronary artery disease. Due to the amount of time a patient has these conditions they tend to build up a large history and often have complications with other conditions. As such the management of these patients often requires extra care. This area is designed to present a clear overview of a patient's record for a condition(s) and make it easy to record new records about any chronic conditions.

Accuro's Chronic Condition management links patient records (including notes, labs, medications, appointments, etc) with a problem history entry. Due to codes used for problem histories there are often multiple codes for the same basic chronic condition and a patient can sometimes have more then one. Because of this a level of grouping above the problem, called the chronic condition encapsulates all the codes.

Chronic Condition (or Disease) Managements is often abbreviated to CDM.

Overview

The header section of the tab, allows for selection of the patient, condition and problem. In addition you add addition conditions or link problems to conditions by using the "--New--" option in the condition and problem dropdowns respectively. The "Exclude Recall" checkbox is used to specify that the problem should not be included in any recall reports (see below for information on CDM reports).

The top left area of the panel contains complications and co-morbidity conditions. These are other entries on the patient's problem history. Complications are problems where they directly effect or are effected by the chronic condition. Co-morbid conditions are other problems a patient has that may effect treatment of the chronic condition.

The bottom left area of the panel lists any medications and/or surgeries that have been linked to the condition.

The middle area contains, labs, goals, general patient encounter notes and forms, and any care teams assigned to the patient. A detailed discussion of goals and care teams can be found below.

The top right of the panel, lists any CDM worksheets that have been completed for the CDM condition. Note that unlike other chart items, worksheets like directly the the CDM condition, not indirectly through a problem history. A detailed discussion of worksheets can be found below.

The right middle area of the panel lists any appointments that have been linked to the CDM problem.

Finally the bottom right area lists any tasks on the patient. Note that since tasks are not currently linked to problems, all tasks will be displayed here, not just ones related to the problem.

View a Graph Lab Results and Medications Charted Over Time

  1. Click the EMR button on the Accuro Navigation bar.
  2. Select the Encounter Notes or the Chronic Conditions tab.
  3. Click the Graph Button graph button in the Labs section. The Labs Results Graphing window is displayed.
  4. Click the down arrow and select the Diagnosis Filter from the list displayed.
  5. Click the down arrow and select the Lab Tests from the list displayed.
  6. Select from the Show Ref Range, Show Meds, Show Appts and Show Points checkboxes, as applicable.
  7. Click the Page Format button page format and change any of your page settings, as required.
  8. Click the Print button printer icon 3.
  9. Select the appropriate printer and click OK.
